By submitted form you are allowing ALA CARE Health Advisors to call you back at your convenient time. • Our Health advisor and Medical team may call and involved to guide you in the complete cycle of the testing process and will facilitate the diagnosis process for prevention of any disease. • Your health vitals and history will be taken for pre-diagnosis and for suggesting the relevant tests or you can also share your prescription with our medical team. • Once you confirm your booking, a trained Phlebotomists (Sample Collector) will be assigned to visit you on your chosen time and pickup address and for collection of sample. Phlebotomist may also take other vitals if suggested in the pre diagnosis. • Your sample will then be transported to the nearest collection hub maintaining the integrity of the sample and will be centrifuged before sending to the lab for processing. • Once the results are processed in the ALA CARE / Partner Lab, medical team at ALA CARE will verify the results and a smart report including historical trends and vitals’ information submitted by you along with simple interpretations will be shared with you. • Package benefits may change according to the availability of the service provider Cancellations shall be considered only if: A valid request is made in this regard within 30 (Thirty) days of receipt of Preventive Health Care Plan by you. The entire Preventive Health Care Plan Welcome Pack is returned to Ala Care within 14 (fourteen) days of receipt of the Cancellation request as aforesaid. For calculation of the aforesaid periods, the records of dispatch, delivery and communications available with ALA Care shall be considered final. No Cancellation shall be carried out if you have purchased the Preventive Health Care Plan under any celebratory / festive offer, or any special / discounted offer of any kind whatsoever. No Cancellation shall be carried out if you have purchased the Preventive Health Care Plan under any interest free easy monthly instalment (EMI) option. No Cancellation shall be carried out if you have in any way availed any of the benefits available under the Preventive Health Care Plan prior to confirmation of Cancellation by ALA Care. In the event a request for Cancellation is received by after dispatch of the Preventive Health Care Plan Welcome Pack to you, OnePlus Health Service shall deduct an amount of 20% of the membership fee and only the balance shall be refunded to you as part of the Cancellation. In the event a request for Cancellation is received by ALA Care after a positive welcome call to you, ALA Care shall refund up to a maximum of 50% of your Membership Fees. Processing fee charged under the ‘EMI Option on Credit Cards’ program is non-refundable. All Cancellations as approved by the ALA Care management as per their discretion shall ordinarily be processed within 21 days of receipt by ALA Care of the Preventive Health Care Plan Welcome Pack. In the event the entire Welcome Pack is not received by ALA Care within 14 (fourteen) days of receipt of a valid request for Cancellation by ALA Care, shall deduct an amount of 50% of the membership fee and only the balance shall be refunded to you as part of the Cancellation. Credit of the Membership Fees to your bank account / credit card account upon Cancellation and refund by ALA Care shall be as per policies and timelines followed by the banks involved and ALA Care shall not be responsible for any delays in this regard. All Medicals Bills to be sent within 7 days from various Medical Practitioners on procuring them. Any medical bill sent by consumer after 7 days shall not be entertained You can avail of services such as medicines, doctor appointments and more, worth your defined benefit every month. Anything you spend over and above, will be charged to you •. If you cancel your subscription any time before completing the 12-month period, your account will be suspended. Order must be placed within 30 days from the date of completion of treatment. However, the company may at its absolute discretion, consider waiver of this condition in extreme cases of hardship where it is proved to the satisfaction of the company that under the circumstances in which the member was placed, it was not possible for them or any other person to give such notice or request benefits within the prescribed time-limit • The extension of subscription benefits is contingent upon their fair usage by subscribers for their own personal purposes • • Monthly fair usage benefit utilization limit for OPD orders is capped at Rs. 1500 and any order above this may be cancelled or flagged for independent doctor verification • Service delivery: Free preventive health check-up voucher can be claimed through our mobile application or web portal Free online consultation shall be provided by designated health care professional of ALA Care. Free online dietician shall be provided by designated dietician of ALA Care. Free online physiotherapy sessions shall be provided by designated health care professional of ALA Care through mobile application only. Free ambulance shall be provided to consumer by ALA Care or reimbursed up to maximum Rs. 500/- Discount shall only be applicable if diagnostic services is delivered by ALA Care certified Diagnostic centre and same can be book through ALA Care Mobile app or web portal, Discount on OPD shall be subjected to post verification of bills will be maximum of up to 50% of the OPD bill values. Discount shall only be applicable if medicines are ordered through ALA Care Mobile app or web portal. Medical loan or pre-approved credit facility shall be Subjected to consumers CIBIL record.
